Azithromycin vs Amoxicillin: three-day pack or the classic course — which antibiotic when?
The short answer
These are different tools, not rungs on a strength ladder. Amoxicillin remains first choice for ear, throat, and many chest infections. Azithromycin's three-day convenience is real, but its best roles are specific: penicillin allergy, 'atypical' chest infections, typhoid, and certain STIs — ideally chosen by a clinician, because casual use is burning through its usefulness.
The Z-pack's short course has made azithromycin dangerously popular for self-medication in Nigeria — three tablets feel tidier than a week of capsules. But short course doesn't mean stronger or broader: each drug covers a different spread of bacteria. Amoxicillin (a penicillin) is excellent against the usual culprits of ear, throat, sinus, and community chest infections. Azithromycin (a macrolide) reaches organisms penicillins miss — the 'atypical' pneumonias, typhoid strains, chlamydia — and conveniently lingers in tissue for days after the last tablet.
That lingering tail is also its weakness: days of low antibiotic levels are a training ground for resistance, and azithromycin resistance in Nigeria is rising in exactly the infections where the drug matters most, including typhoid. Neither drug touches malaria or ordinary viral colds — the two conditions for which Nigerians most often buy them.
